How to be removed from our registry and database
Registration on CHS and participation in our surveys is voluntary. You can
remove your information from our database at any time. Once it has been
removed, you will no longer receive communications from us. To be removed
from our database, please contact us at info@canadianhealthsurveys.ca. Please note that it may take up to seven business days to process your request.
