CanadianHealthSurveys.ca (CHS) respects the privacy of every individual who visits our website and protects your personally identifiable information. This privacy policy outlines the information we will collect and how we will use that information. CHS considers respondent privacy as part of our business ethics. Our privacy policy is based on the most stringent federal legislation and Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).

CHS will not collect any personally identifiable information such as your name, address, telephone number, or email address unless you provide it to us voluntarily. We do not knowingly collect information from users who are under the age of 18. We will not share your personally identifiable information without your knowledge or permission. We do not require your mailing address during our registration process. At a later date, we may request this information for the purposes of specific surveys in which mail may need to be sent. In these cases, your participation and providing your mailing address is optional. When you do provide us with personally identifiable information, we may use that information in the following ways unless stated otherwise:

  • We will store and process it to better understand your needs and how we can improve our partners products and services. We will make reasonable efforts to keep your information accurate and up-to-date based on the extent of the information you provide and the frequency of your participation in our studies.
  • We may use it to contact you, to send you additional information or surveys about your illness or condition. We will request your permission before sending anything and will never ask you for money or attempt to sell you anything.
  • We will not provide or sell your personally identifiable information for marketing purposes or any other purposes to other organizations/third parties outside of our database management partner.
Your personal information is securely stored and protected.

How to be removed from our registry and database
Registration on CHS and participation in our surveys is voluntary. You can remove your information from our database at any time. Once it has been removed, you will no longer receive communications from us.
